  • Pills: The most traditional form.
  • Patches, Gels, Sprays (Transdermal): These deliver hormones through the skin, bypassing the liver, which may reduce the risk of blood clots compared to oral estrogen.
  • Vaginal Rings, Creams, Tablets: These are used for treating local symptoms of GSM and have minimal systemic absorption.

Types of HRT: Estrogen-Only vs. Combined Therapy

This is a critical safety principle. According to ACOG guidelines, if you have a uterus, you must take a progestogen (like progesterone) along with estrogen[9]. Unopposed estrogen can stimulate the growth of the uterine lining (endometrium), increasing the risk of uterine cancer. Women who have had a hysterectomy can safely take estrogen-only therapy.

Bioidentical vs. Synthetic Hormones: What’s the Evidence?

You’ve likely heard the term “bioidentical hormones” (BHRT). This means the molecular structure is identical to the hormones your body produces. Some FDA-approved HRT products (like Estradiol patches) are bioidentical.

The controversy arises with custom-compounded BHRT. These are mixed at special pharmacies based on a doctor’s prescription. While proponents claim they are more “natural” and can be tailored to an individual, major medical bodies like The Endocrine Society and NAMS have expressed safety concerns. Custom-compounded hormones are not regulated by the FDA, meaning they haven’t been tested for safety, efficacy, or consistent dosing [13].

The choice to use HRT is deeply personal and should be a collaborative conversation with your healthcare provider—a process known as shared decision-making. As Dr. Jessica L.
