Physical Therapy & Massage Therapy New Patient Visit: 60 mins
Functional Medicine New Patient Visit: 40 – 60 mins.

How do I know what my benefits are?
How do I know how much I will be paying before my appointment?
1. Your treatment and your insurance. Treatment plans are different per patient depending on what you and your provider discuss in your appointment.
2. It depends on if you have copay, if your subject to the deductible, and how much your coinsurance covers.
We can provide this information for you one you complete your insurance verification form.
Who do I contact if I have a billing question?
If I go to multiple Vida clinics, am I able to have one bill or do I need separate bills per clinic?
Not sure about your insurance?
Vida accepts all major insurance providers, and all motor vehicle insurances. We take the guess work out of your insurance benefits.
We will verify your insurance for you and let you know what coverage you have for our services prior to your first appointment by filling out the our online Contact US Form and our online scheduling coordinator will contact you with all of your insurance coverage details.
We do not accept Medicaid, Molina or Apple Health plans at this time.
Insurance coverage depends on if the Provider is in network with the insurance carrier, not Vida. Most Providers at Vida are in network with the following insurance carriers…
